Supporting Evidence-Informed Community Care for Stroke Survivors with Multiple Chronic Conditions through Mobile Health Technologies

Mobile technologies offer a practical strategy to move best practices into health care provider routines to improve health care systems and client outcomes in an acceptable, convenient, timely and efficient manner. We designed and built an innovative mobile health (mHealth) solution to support community health care providers in providing evidence-informed care to older adults with multiple chronic conditions (MCC) who have experienced a stroke.

In using a qualitative descriptive design for this study, end users were engaged in the design of the mHealth application. The mobile application was web-based with a mobile phone platform. We aimed for this information to be accessible by health team members, and provided key metrics, statistics and summarized information in the form of standardized reports to the interprofessional team as needed.
