Promoting Independence in Older Canadians with Diabetes

Diabetes Action Canada has a new bilingual website and ACHRU is featured as one of their programs of research. You can find our ACHRU Community Partnership Program for Diabetes Self-Management for Older Adults – Canada under the ‘Promoting Independence in Older Canadians with Diabetes’ button on the home page of their new website.

This innovative community-based model for diabetes care is a six-month multi-faceted, evidence-informed intervention designed to integrate care across settings to improve the quality of care and health outcomes in community-living older adults (i.e., people ≥ 65 years) with diabetes and at least one other co-morbid condition, and to support their family/friend caregivers.

The program is funded by the Canadian Institutes of Health Research Pan-Canadian SPOR Network in Primary and Integrated Health Care Innovations (PIHCI), Diabetes Action CanadaMcMaster Institute for Research on Aging (MIRA), and Scarborough Health Network.
