New Report: My Stroke Team (MyST) Knowledge Exchange Event

The My Stroke Team (MyST) Knowledge Exchange Event brought together researchers, health care practitioners, patients, decision-makers, funders, technology experts and a privacy officer to exchange ideas, evidence and expertise. Study results, patient and health care team perspectives were shared, along with a case study, lessons learned and future directions.

MyST is a new way of providing outpatient stroke rehabilitation services for community-dwelling older adults with stroke and multiple chronic conditions. This approach involves a six-month interprofessional stroke team. The team provides: regular in-home visits and conducts monthly case conferences, supported by a web-based application (app) MyST (My Stroke Team).

David, a MyST study participant, was happy to share his experience at the MyST Knowledge Exchange Event one year and seven months after his stroke. “I can’t tell you how fantastic the (MyST) program was for me.”
