ACHRU Receives Poster of Distinction Award at HQT2016

Congratulations to ACHRU’s Amy Bartholomew and Della Hare from Ross Memorial Hospital on receiving a poster of distinction award at Health Quality Ontario’s Health Quality Transformation (HQT2016).
Their poster, Community Partnership Program for Older Adults with Diabetes and Multimorbidity: A Pragmatic Multi-Centre Randomized Controlled Trial, was well received at the fifth annual interactive conference for patients, caregivers, health care providers and system leaders.
Results from ACHRU Core study 6 - A Client-Driven Intervention to Support Self-Management among Community-Living Older Adults with Type 2 Diabetes Mellitus and MCC - support the effectiveness of the intervention in improving health-related quality of life in older adults with multimorbidity in community settings. This will help inform the development and dissemination of a new and innovative community-based model for diabetes care that can be adapted and implemented for older adults with T2DM and MCC across Canada.
