- Understand how older adults with MCC, family caregivers and health professionals manage MCC;
- Examine the factors, health outcomes and costs associated with MCC;
- Examine the feasibility and acceptability of multi-component interventions to promote health and/or self-management of chronic conditions, such as integrated interdisciplinary teams, volunteer-based peer support, patient navigators, and mobile or internet-based support; and
- Determine the effectiveness of the interventions on health-related quality of life, safety, use and costs of health-care services, and other client and caregiver outcomes.
The objectives are to:
