New Infographics for the Community Partnership Program for Diabetes
Researchers, patient/public partners, and a graduate student from the ACHRU’s Community Partnership Program (CPP) for Diabetes team have created six new infographics about older adults with diabetes from an analysis of population-based data.
The infographics, based on data from Ontario and Alberta, take a glimpse at older adults with diabetes who are high emergency department users, their socio-demographic characteristics, health, and prescription medications, and use of other health care services.
They are based on data from the CPP program’s Study 1 – Population-Based Study, led by Drs. Andrea Gruneir and Kathryn Fisher. This study aimed to identify the socio-demographic, health and health service use characteristics associated with high use of the emergency department among older adults with diabetes. The analysis uses health administrative data from the provinces of Ontario and Alberta.
Study 1 results provides a better understanding of high emergency department use among older adults with diabetes and how this information can inform recruitment into the CPP Randomized Controlled Trial (Study 2) and plans for scaling up the CPP intervention.
The CPP trial, led by Drs. Maureen Markle-Reid and Jenny Ploeg, is designed to evaluate the effectiveness of the CPP in diverse multi-jurisdictional, culturally and geographically settings In Ontario, Quebec and PEI. This community-based model for diabetes care aims to improve diabetes self-management and the quality of care for community-dwelling older adults with Type 2 diabetes and multiple chronic conditions (MCC).
